〔Abstract〕 Diabetes self-management education and support (DSME/S) is a very important part of diabetes prevention and control, and all patients with diabetes should receive diabetes self-management education to improve diabetes outcomes. Shenzhen is one of the earliest cities to carry out community health services. Diabetes management has been transferred from hospital to community in more than ten years, but it is still far from the goal of "Same-city and Same-quality" diabetes management with the integration of medicine and prevention. In order to continuously improve the low "three rates" (awareness rate, treatment rate and standard reaching rate) of diabetes and reduce the disease disability rate and mortality rate, a series of action strategies were proposed based on the needs and practice progress of community management of type 2 diabetes in Shenzhen, mainly focusing on the construction and implementation of "Shenzhen Community Diabetes Specialist" talent training system. To carry out voluntary diabetes screening in the community, standardize the integrated diagnosis and treatment behavior in the community, and promote the self-management education and support for diabetes in the community. This paper introduces the practice and thinking of DSME/S in Shenzhen community in order to promote the quality management and standardization education of diabetes mellitus in Shenzhen.
